 GENERAL REGISTRATION INFORMATION:

Regular Registration will close on Saturday, February 6th.  After that date, there will be a $15.00 LATE FEE.  Returning players must be registered by the close of regular registration to be guaranteed to return to their fall 2009 team.

Registration fees are $90.00 and this includes the uniform. Players that will play in the U15 and older age groups are $105.  There will be a $25.00 fee for any returned check and payment must be in cash.  ALL NEW PLAYERS MUST BRING A COPY OF THEIR BIRTH CERTIFICATE.  If registering online, please mail the birth certificate to the address below.

Only the Under 5 and Under 6 players may request a specific team or coach.  CISA does not allow older players to be placed on a specific team.  This is done to ensure that our coaches are not recruiting to build a strong team.  CISA provides recreational soccer that allows players to have fun and learn the game.

Teams will be formed and given to the coach at the coach's meetings the second week of February.  Your coach will then contact you for the first team meeting.

First games will be March 6th.  The season will run until mid May.

CISA has all volunteer coaches.  Please let the appropriate board member know if you can coach.  This is especially important in the Under 5 and Under 6 age groups.

REGULAR REGISTRATION CHOICES

ONLINE, has 2 choices.  If your child played in the fall of 2009, use the returning player link.  All of your previous information will appear and you only have to make changes.

If your child did not play in the Spring, use the new player link.  Please complete all information.   

If you wish to play up one year, please list that information in the comments section.  If you wish to not play on your previous Spring team, please state this in the comments section and you will be placed in the draft for a new team.

Returning Player Registration – This link brings you to your Member Profile log in screen.  We urge you to use your log in information from last season to avoid creating duplicate records.  If you do not have your user information, you can select the ‘I don’t know my Username/Password ‘ link to retrieve it via email

In person registration will ONLY be the following dates, times, & locations:

Saturday, February  6^th from 11-2 pm at Walgreens at Sycamore School Rd and McCart, and Cousin’s B-B-Q in Crowley. 

  New players must bring a copy of their birth certificate.

Click on the Registration form link below, print, and fill out completely. Enclose payment and birth certificate copy, if applicable.  Mail to: CISA, P.O. Box 894, Crowley, Texas 76036.  The registration must be received by February 1^st.  If you are a new player, a copy of the birth certificate must be included.

ATTENTION:
This is the link to the coaches and volunteer background check site.  This is a free service and must be completed by each coach, assistants, team managers, and volunteers before your team roster will be made available.  Please take the 5 minutes to complete this registration, so that your team roster will be available to you at the coaches meeting. If you completed this in the fall of 2009, you do not need to do again.
